Our forms can be downloaded below. They are in PDF format so you will need Adobe Acrobat (Version 6.0 or later) to open the files. If you do not have Adobe Acrobat Reader and would like to download it, please click on the link at the bottom of this page.
You will be able to fill out the forms online. After you do so, you can print the forms out and bring them to the clinic. Forms 1 through 3 are required to be completed and signed prior to starting treatment. You may fill out form 4, the Client Satisfaction Survey, whenever you'd like and as often as you'd like.
